You can add images into a form if you need to. They have to be manually entered using html code. the code is as follows:

You must first upload the photo you want to use in the documents section of you account to obtain a direct link to the photo> Go to MENU> FILES> “Documents” and upload the image(s) you want to use. One the image is uploaded, click on the image link and copy it from the address bar in your browser.

Add an "editor" field to your form where you would like the image to go. Click on the field and click the HTML tab at the top right of the text editor.

Add the code as shown above and copy and paste the direct link to the image you obtained in the documents where it says INSERT IMAGE LINK HERE inside of the quotation marks. Save changes.

You may then resize the image in the VISUAL tab of the text editor.
